Personalized Comprehensive Initial Assessments

We provide a comprehensive, personalized initial assessment that includes an in-home or virtual visit by our Registered Nurses with case management certification.

Our assessment includes a formal, written evaluation highlighting the current services in place, support system, current medical team, medications, medical equipment, home layout, insurance coverage, supports already in place and more.

The assessment concludes with recommendations that may identify gaps in care, insurance coverage, home health, additional healthcare providers, community resources, assistive devices for necessary home modifications and appropriate level of care.

You may hire us to implement the recommendations once the assessment has been completed, or not.

This initial assessment is often requested by family members and loved ones concerned about their loved ones’ current safety, functional status and additional services that may be needed to remain in the home.
