Adrienne Bannon, founder of Concierge Healthcare Coordination, is a Registered Nurse and Certified Registered Nurse Case Manager (CCM) committed to supporting patients, families, and professionals as they navigate the often overwhelming and fragmented healthcare system.

After 36 years of working in healthcare in a variety of settings, Adrienne is well versed in the flaws and frustrations patients and families often experience regarding the delivery of quality and coordinated healthcare.

This business was born from witnessing the frustrations surrounding gaps in care and the desire to alleviate the stress often experienced when coordinating care, hospital discharges, and services.

Our mission is to provide a clear and concise picture of where our clients are in the healthcare continuum, followed by professional recommendations.
