Healthcare decisions often happen during stressful moments. Families may be dealing with hospitalization, discharge planning, changing care needs, insurance questions, safety concerns, or uncertainty about what level of care is truly appropriate.
Concierge Healthcare Coordination provides experienced, nurse-led guidance so clients are not left trying to manage those decisions alone. We assess the situation, identify gaps, coordinate communication, and help create a clear path forward.
We do not provide hands-on medical care, diagnosis, treatment, or medication administration. Our role is healthcare coordination, assessment, advocacy, education, and ongoing oversight.

Founder Profile
About Adrienne Bannon, BSN, RN, CCM
Adrienne Bannon is a graduate of the Johns Hopkins School of Nursing and has proudly maintained her Certified Case Management (CCM) credentials since 1996.
With 36 years of experience in nursing and case management, she brings a wealth of knowledge spanning hospital care, insurance systems, community health, homecare, end of life care, and independent consulting.
Throughout the years, Adrienne has supported patients and families through complex medical transitions. She has worked closely with attorneys and trustees in matters involving guardianship and special needs trusts.
Her steady, experienced approach ensures that clients receive thoughtful guidance and comprehensive oversight at every stage of care.
